Optimizing Medical Floor Staffing to Reduce Turnover and Protect Ratios
VP of Medical Floor Recruitment
I talk to hospital CNOs every week, and the conversation always circles back to the same number: patient-to-nurse ratio. When a medical floor is running 1:7 or worse, the problem is not the patients. The problem is the staffing pipeline. Specifically, the problem is what happens — or does not happen — in the first 48 hours of a vacancy.
Effective floor staffing starts with proactive sourcing. That means engaging passive Med/Surg, Telemetry, and PCU nurses before a vacancy opens — not after. A warm pipeline of experienced, actively licensed nurses who already know your unit type, region, and compensation range eliminates the weeks of cold list-building that inflate ratios and accelerate turnover. Plenty of them hold CMSRN or PCCN credentials — a nice bonus, but a clean license and real floor experience are what we screen on first.
The second piece is acuity matching. A nurse who thrived in a low-acuity med-surg may struggle in a high-acuity PCU or telemetry step-down. When we screen floor nurses, we match unit acuity to stated experience. This is not about overriding the hiring facility. It is about presenting candidates whose background aligns with the unit's actual patient mix so interviews are productive, not exploratory.
Finally, retention analytics. I see too many medical floors tracking turnover as a single number. That is useless. You need to break turnover down by unit, by shift, by tenure, and — most importantly — by reason. A voluntary exit for relocation requires a completely different fix than a turnover driven by ratio strain. Until you can see that breakdown, you are treating symptoms, not causes.
When we evaluate floor nurse leaders for our hospital clients, we look for people who can walk into a unit, audit the current staffing model, identify where the ratio breakdown is happening, and redesign the process. That is the leader who turns a 1:7 floor into a 1:5 floor. That is the leader worth fighting for.
